Clusters of microembolic signals: a new form of cerebral microembolism presentation in a patient with middle cerebral artery stenosis.

چکیده
BACKGROUND Middle cerebral artery (MCA) stenosis is a rare occlusive disease in western populations, with a high risk of stroke recurrence. An artery-to-artery embolic mechanism has been argued. We report the detection of a new pattern of microembolic signals (MES) in the MCA poststenotic segment in a patient with multiple recurrent transient ischemic attacks. CASE DESCRIPTION A 75-year-old man was admitted to our hospital with a transient leg monoparesis on the left side. Right MCA stenosis was detected by transcranial Doppler ultrasonography (TCD). In spite of aspirin treatment, the patient had a recurrent right hemispheric transient ischemic attack, and anticoagulant therapy was started. A new, similar event happened after correct anticoagulation. The bigated TCD monitoring study of both MCAs disclosed clusters of MES at the poststenotic MCA segment. Each cluster contained between 12 and 45 embolic signals. The patient became asymptomatic and clusters of MES disappeared, coinciding with the combination of ticlopidine and oral anticoagulants. CONCLUSIONS We describe a new presentation of MES, ie, grouped in clusters of signals, that may be associated with a high risk of stroke recurrence.

Cerebral microembolism in patients with retinal ischemia.
BACKGROUND AND PURPOSE We investigated the frequency of cerebral microembolism detected by transcranial Doppler ultrasonography in patients with clinical evidence of retinal ischemia, including transient monocular blindness, central and branch retinal artery infarction, and ischemic oculopathy, and assessed its correlation with carotid artery stenosis. METHODS Records of 331 consecutive patie...
متن کاملMicroembolic signals and risk of early recurrence in patients with stroke or transient ischemic attack.
BACKGROUND AND PURPOSE Asymptomatic microembolic signals (MES) can be demonstrated in patients with cerebral ischemia using transcranial Doppler (TCD) ultrasonographic monitoring of the middle cerebral artery. However, the clinical relevance of MES remains uncertain. The purpose of this study was to estimate the independent contribution of microembolism to the risk of early ischemic recurrence ...
متن کاملMicrobubbles detection during cardiopulmonary bypass with transoesophageal echocardiography: a case report
INTRODUCTION Microembolic signals are usually detected with transcranial doppler during cardiac surgery. This report focuses on suggesting the transesophageal echocardiography as a different diagnostic approach to detect microemboli during cardiopulmonary bypass. CASE PRESENTATION A 58 year old male patient, caucasian race, was operated on video assisted minimally invasive mitral valve repair ...
متن کاملThe Frequency of Cerebral Microembolism in Acute Myocardial Infarction
ABSTRACT Introduction: Stroke is more common in patients with cerebral microembolisms. Frequency of cerebral microembolisms (high intensity transient signals, HITS) in acute myocardial infarction has been reported about 17%. The factors that influence on microembolism after myocardial infarction (MI) are not definitive. Type of MI, Ejection fraction, Hx of Streptokinase is the factors that were...
متن کاملEmbolism distal to stenosis of the middle cerebral artery.
Fibrin-platelet microembolism in cortical branches distal to stenosis of the middle cerebral artery was directly visualized in a patient with transient ischemic attacks (TIA) and was histopathologically confirmed. Cerebral microembolism may produce TIA in patients with stenosis of the middle cerebral artery and may influence the success of the extracranial-intracranial bypass operation in treat...
